31.01.18. the report on the implementation of the Roadmap for improving the children's oncology and hematology service in the Republic of Kazakhstan in the selector with the Ministry of Health, the Scientific Center of Pediatrics and Children's Surgery and the National Research Center for Motherhood and Childhood was held. In the discussion of the report, non-governmental organizations, patient organizations, regional health authorities and other medical organizations took an active part.
At the end of 2017, 610 children with malignant neoplasms were first diagnosed in the republic (an average of 600 children per year for the first time). In general, there is a slight increase in oncological morbidity in children, which is more likely due to improved diagnosis. Among oncological pathologies in children, as in the whole world in Kazakhstan, the first place in the structure of acute leukemia (39%), followed by brain tumors (16%) and neuroblastoma (13%). In the age structure of about 40%, young children become ill, according to world statistics, these are mainly tumors of an innate nature. It is well known that the diagnosis of cancer at early stages predetermines the results of therapy, therefore in the republic in recent years a lot of work has been done to train primary care physicians on alertness and to raise awareness of the population about the initial symptoms of diseases. So the diagnosis of childhood cancer in stage 1-2 has improved since 2015 by 11.2%.
Within the framework of the Road Map for the Children's Oncology and Hematology Service, a number of regulatory and legal acts were amended, some clinical protocols were revised, a lot of methodological work was done to introduce into the practical healthcare of the regions, republican centers providing assistance to children with oncopathology equipped with modern diagnostic and therapeutic equipment and measures were taken to develop transplantation and rehabilitation.
According to the results of the meeting, it was noted that over the past year, the survival rate of children with acute lymphoblastic leukemia increased to 78%, with lymphomas up to 79%, renal tumors up to 82%, retinoblast up to 92%, bone tumors up to 90%, but the results of therapy of tumors brain, neuroblastoma, acute myeloblastic leukemia. Mortality of children with cancer in the 2 republican centers, where the main treatment of these patients is carried out, was 2%. Despite this, the death rate due to oncological diseases in children is not leading in the overall structure of infant mortality in the country. The first place in the structure of infant mortality is the state of the newborn period and the second place is congenital malformations.
With the beginning of bone marrow transplantation in children from 2012 to 2017, 107 transplantations were performed in the republic, 47 of them were performed in the past year. In children with bone tumors, such operations as endoprosthetics, which allow to preserve the limb affected by the tumor and improve the quality of life of the child, are increasingly being performed.
The further application of modern complex programs of therapy of tumors in children, new methods of surgical treatment and radiation therapy will allow to further improve the survival rate of children with malignant neoplasms, and with some cancers, to achieve a complete cure. In our time, a child's cancer is not a verdict, but only a serious diagnosis that requires timely diagnosis, joint efforts of the patient's specialists and parents.
